Scoring Error

My catcher make a throw in an attempt to prevent a stolen base, and the runner is safe, the second baseman was not positioned to catch the ball and the short stop backing up the throw allowed the throw to go through into center field that should have been backed up with "ordinary effort."  The runner takes an additional base (third base) due to poor fielding of the throw, neither the second baseman nor the shortstop touch the ball.  The centerfielder's lack of effort in backing up the throw down to second allowed the runner to advance another base and score from third.  I scored the play as follows:
Runner advanced to 2nd on stolen base
Runner advanced to 3rd on E6 (even though he did not touch the ball)
Runner advanced home on E8 for not backing up the throw to second and allow the runner to advance an extra base home.
Is this correct, can I score a player an error for allow a runner extra bases for not backing up plays?  If not how should this be scored?

Based on your note, I see the short stop is credited w/ one error, however, it's a two base error....Not two separate errors.....
