Hey, I wanted to first thank you for the tip about the left hand from a few weeks back. My drives are going a lot straighter now.

I just finished up a tournament today and shot 83. It was a good round overall, but there were a few things that I think if I nail down I can easily shoot in the mid-high 70s, CONSISTENTLY.

1. Driver - Right now my miss hit is a high weak fade/slice. My swing thoughts for the driver are usually just short backswing and get the body through the ball. Today, the front nine most of my drives were fading but I figured it out by the back nine. Still, it cost me easy 4-5 strokes. GIR on front vs back was 6 to 2 ... have any suggestions for other swing thoughts to keep the ball straighter?

2. Putting - I could not make any putts outside of 10 feet. I fixed up my yips with the 4-5 feet putts that I've been having, but I had SO many opportunities for birdie on the back 9 today, from around 10-15 feet. I need to make some more of these birdie putts... have any suggestions for more effective ways to practice putting?

Nice.  Glad it helped.  If you are fading/slicing, allow the arms and club to swing past your body.  Instead of trying to get the body through the ball, quiet the body and let the arms, hands and club swing past the body.  Your body will do just fine if you leave it alone.  This will help you square the clubface up a little more.  

Putting:  I would spend a lot of time checking your alignment.  I would practice those 10-15 footers with 2 shafts on the ground to line you up.  Barely give yourself enough room on either side of the toe and the heel, make sure the shafts are lined up to where you need to start the putt, and work on rolling the putt the correct distance.  Let the break of the green take care of the putt.  Your job is to line it up correctly and then roll it where you're looking.  If you don't line it up correctly, it's hard to get any consistency in putting.
