Possible need for expert witness and a question

A 12 year old playing in his first tackle football game suffered a concussion which has had long term effects. Video of the game shows him trying to tackle a running back at the line of scrimmage helmet to helmet. Although the rest of the team had practiced both with and without pads, the coaches never gave him pads to practice in.  The day of the injury was the first time in his life he had ever played in pads or a helmet (also the helmet was never inflated). I have read a fair amount of league regulations that require a school age player to practice for 10 days (give or take) without pads before scrimmaging, but I can't find many regulations that require a certain number of practices in pads before playing in an actual game.

Is there a recognized standard that would require practicing in pads?  

Is there a correlation between not practicing in pads and an inability to avoid a helmet to helmet tackle?

As a follow up to my last reply, here is a website that discusses the proper fitting of equipment.  http://www.skylinehospital.com/pt/Sports/Coaches_Corner/Football/Equipment_Fit/e  This may be a factor in your situation.  The National Athletic Trainers Association has several documents that address this issue and injury prevention.  Also Instructions for Football Pads use is a duty of a coach.  It could be delegated to a trainer or equipment manager had higher levels of play and organization [college, professional, etc.)
