I'm sorry for the delay, I didn't get this question and have to check manually.  The last few days I am getting questions so I think the problem is fixed.

1-no
2-1
3-batter is out ball remains alive
4-yes
5-8 or 9
6 a starter can be subbed, re-enter (in the same batting position) and subbed again, they are now out of the game.  a sub can enter, be subbed for and re-enter 1 more time (in the same batting position).  If the are subbed for again now they are out of the game.
