The libero can replace any back row player. Currently I have my libero replace
my middles when they get to the back row. My question is can I start this way
and then change during a set and leave a middle in the back row or sub for
her and then put my libero in for another back row player? In more general
terms, can the libero go in and out for any back row player at any time in a
set for any backrow player or is the libero limited to the rotation replacement
system that you start a set with?

You sound like you know exactly what you're doing!  And everything you say is correct.

But let me make sure we're clear.  So at the beginning of a set, libero #8 replaces MB #12.  Then, later in the game, after MB #12 enters the back row for the 2nd time, you can make a substitution with DS #3 for MB #12.  And you can have your libero replace a different back row player.  

Now, what's illegal?  (I think you're already aware of this, but I'm going to mention them anyway for other readers.)
a) Change liberos during a set, unless of an injury.
b) Have the libero come out, then w/o waiting a rally, immediately go in for another player.  
c) Allow the libero to enter the front row.
