fishing knots

Question
While fishing the other day, I kept loosing my hooks. I was doing a basic fisherman knot, wrap around 6 times and pull through, what was I doing wrong? The line came undone, every time. lost 4 big salmon.
What was I doing wrong?

If you are using monofiliment use the Improved Chinch Knot:

If you are using one of the new super braid lines a chinch knot can pull out and I perfer a Palomar knot.
