I am needing a replacement license and certificate. My card is worn out. thanks, Doug.

Hi Doug,

You didn't say which agency your were certified under so I cannot give you a specific procedure for replacement of your certification card.

However, all of the major certification agencies have procedures for the replacement of a lost or damaged certification card.  You will need to contact that specific agency to request a new card.

The easiest way to reach your agency is to Google the name and follow the link.  For example, you can Google "PADI", "NAUI", or "YMCA Scuba" and then click on the link provided.  Generally, you will have a form to print, fill in your information and then send in the form along with a picture and a processing fee.  After researching their database to be sure you're registered with them, the agency will send a replacement card to you.

Some agencies, PADI for example, will permit you to replace your card through any of their licensed dive centers.  You can go to the dive center and they can immediately verify your certification through their proprietary internet links.  Then you can have your digital picture taken, pay the replacement card fee, and the card can be sent directly to you.

Doug, in your question, you referred to your "license and certificate".  The license is your certification card (replaced as described).  Your certificate is usually a form which contains your handwritten name and is signed by your instructor.  It does not contain your diver registration number and is not generally accepted as proof of certification.  The certificate is not generally replaced but it can be done if you wish.  Again, contact your certifying agency or visit one of their licensed dive shops and ask for a new certificate.  The certificate can be filled out after verifying your status.

Once again, Doug, this information is general since you didn't tell me the name of your certification agency.  However, I think it will provide you with enough guidance to successfully have your card replaced.
