12 ga. single shot

Hello Bob, I have a single shot 12ga that belonged to my wifes great grandfather. I can clearly read The Cresent FireArms Co. Norwich,Conn. Ican also read ****15 Empire Ejector. It appears that something is before the 15 but cannot read it. The serial numbers on the barrel and under side of receiver portion reads 751638. I would like to know more about the gun and can you still get replacement parts and where?
