Rope and Belay Breakage in Rock Climbing

Question
Hi.  I am doing a piece of work and I am required to find cases where the belay device or the rope has broken.  Would you be able to provide me any incidents that you know of in this situation?

Answer
I personally have never experienced this.  The only incident that I can think of was involving a slack line made with webbing that had been exposed to the elements for much too long.  The line broke, and the man fell to his death, as it was quite high in the air.
