I am trying to find a form that is generic (or not) that can be used to show transfer of ownership. I wish to be able to provide something to the state that I am in (NV)to show that I have released my rights to a firearms (shotgun)so that the new owner can register it in his name. It would be nice if it included a notory area for documentation and be kept on file by me if needed for future use.

I have a form that you can probably use.  If you send me an email at:

[email protected]  (remove the -spambuster before sending)

I will email you a form that works for most people.  

Also, just a set the record straight, Nevada does not require firearms registration.  There is a local provision for registration in Clark County, but otherwise it is not required.  The form I use does not need to be notarized, but does require that the purchaser provide his or her drivers license info.  This is usually adequate to establish identity.
