HI Dr George Tsatsos,

I was quite a good runner, with lots of experience, with lots of half marathons and 5 marathons. I ran each week and was in a running club. I think broke a toe, which took sometime to get over,  3 months. Somehow after this I damaged both my Achilles, they had big lumps on the back. They are now constantly sore, and inflamed, and this is making my return to running tricky. I have tried regular sports massage, to know avail. I was wondering what else I could do, ie surgery etc. Any advice to get me back running would be very helpful. I am mid 40'5 now.

Dear Simon
The lumps in the back of your heels are probably small tears of the tendon from the calcaneous (heel bone). Since the bone does not know any better-it interprets the tears as small bone fractures and tries to repair them with more bone-hence the bumps.
Try gentle extended stretching of your Achilles tendons and possibly a night splint.
The clicking noise in your ankles may be due to tenosynovitis of the tendons that pass around your ankle. These tendon inflammations take time to quiet down-and may benefit with physical therapy.
Try warming up your ankles with ankle rotations and stretches before your running.
For now try PRICE
Protect-Wear an ankle brace around the ankle and limit the movements that aggravate it.
Rest-Speaks for itself--rest it.
Ice- Will help if the ankle is painful.Ice after running-lots and lots-for about 45-60 minutes
Compress-Ace wrap  or ankle sleeve
Elevate-Will help if the ankle throbs or swells at the end of the day.
